This resource is an example of Neighborhood Health Status Improvement from the Greater Rochester Health Foundation.
It profiles the Seneca Towns Engaging People for Solutions (STEPS) initiative taking place in Seneca County, New York, USA. The project is a partner of the S2AY Rural Health Network, a horizontal network of rural Public Health Departments.
The STEPS project focuses on the social determinants of health – the socially constructed factors that influence health outcomes, such as income, education and housing conditions. The STEPS initiative’s goal is to address the physical, social and economic opportunities in the county and, in doing so, improve the health outcomes of residents.
