Rural Health Hubs Framework & the Collective Impact Opportunity

Developed and endorsed by a multi-sector rural health hub advisory committee, the Rural Health Hubs Framework for Ontario was finalized in January 2015. A central recommendation within the framework calls for the development and implementation of local Community Health Plans in rural communities in collaboration with local health and social service providers and community members.

This workshop, presented at the Association of Ontario Health Centres (AOHC) Shifting the Conversation Conference, introduced participants to Collective Impact as a framework that has demonstrated tremendous success in engaging multiple sectors to effectively address a range of complex community issues.  Participants were then invited to consider how Collective Impact might provide a useful way to develop Rural Community Health Plans and translate the concept of Rural Health Hubs into reality across Ontario.
